Kaiser Home Health Services receives award
Kaiser Home Health Services was recognized for their High Quality of Care and ranked in the top 1% in the Nation. Recently, Kaiser Home Health Services received the Home Health Collaborative Award of Excellence from TMF Health Quality Institute, the Medicare Quality Improvement Organization for Texas.
The Home Health Collaborative Award honors Texas home health agencies that have worked with TMF over the last two years and achieved required levels of improvement in specified quality improvement measures.
Kaiser Home Health Services was awarded the Award of Excellence for reduction in Acute Care Hospitalization with a low score of 3.6%. The National top 20 percentile scores were 22.3 and the national top10th percentile score was 18.8%.
"At Kaiser Home Health Services, we recognize the importance of improving patient care. We are proud that our agency has received this award because showing improvement in these quality measures ultimately means our patients are receiving better care," said Patricia Garcez RN Home Health Director..
Out of more than 1,000 home health agencies in Texas, approximately 400 chose to work with TMF on quality improvement. Of those 400 participating agencies, 12 earned the Award of Excellence and 51 earned the Quality Achievement Award.
Agencies receiving these awards were recognized at a special ceremony in San Antonio. Kaiser Home Health was also listed in the top one percent in the nation in reducing their acute care hospitalizations scores.

"These agencies demonstrate they are dedicated to ensuring all patients receive the right care at the right time, every time."
